Yang, D.V.M. and the staff of Thornhill Pet Hospital, Inc. to examine, prescribe for, treat, or perform surgery upon the pet(s) described on the attached pet information sheet(s). I agree to pay at the time that my pet is discharged for Thornhill Pet Hospital, Inc. or when services are terminated, unless prior financial arrangements have been made. I understand that veterinary service is only provided during nighttime and weekend hours as deemed necessary in the judgement of the veterinarian in charge. Continued physical presence of qualified personnel may not be provided when Thornhill Pet, Inc. is closed.

A small, family owned and run practice that is reasonably priced and very honest. Dr. Yang is friendly, thorough and knowledgeable and is always very sweet to our cat!
